The Skinny on MASLD: From Nomenclature to Novel Therapies

OverviewJoin us for this activity that provides clinicians with a practical framework for metabolic dysfunction-associated steatotic liver disease (MASLD). It reviews the updated nomenclature and diagnostic criteria, a stepwise noninvasive risk-stratification pathway (FIB-4, VCTE, MRE, ELF), the multidisciplinary care model, and the appropriate selection, monitoring, and response assessment of newly approved MASH pharmacotherapies (resmetirom and semaglutide).

1. Apply the 2023 MASLD nomenclature and diagnostic criteria - hepatic steatosis on imaging plus at least one cardiometabolic risk factor - and the alcohol thresholds used to exclude primary alcohol-related liver disease.

2. Perform primary risk stratification with FIB-4 in the primary care setting and determine appropriate reassessment intervals or referral based on low, intermediate, and high-risk results.

3. Interpret secondary noninvasive tests (VCTE, MRE, ELF) to identify clinically significant fibrosis, recognize suspected cirrhosis, and identify when liver biopsy remains appropriate.

4. Select appropriate candidates for MASH pharmacotherapy with resmetirom or semaglutide, applying eligibility criteria, contraindications, and the universal mandates of cardiometabolic risk reduction, alcohol monitoring, and lifestyle management.

5. Implement the recommended safety-monitoring cadence and evaluate treatment response at 12 months (resmetirom) and 72 weeks (semaglutide) to continue, re-evaluate, or discontinue therapy.
