Threshold CoC Street Outreach Council Bluffs

Are you sleeping outside, living in your car, or staying in an abandoned building?

Threshold CoC Street Outreach team offers a variety of supportive and Housing-Based Case management Services for our neighbor experiencing homelessness. They are available Monday through Friday for Outreach and Drop-Ins.

During Outreach, the following services can provide:

- Housing Assessments

- Mental Health Services

- Mainstream to Benefits

- Referrals to Shelters

- Substance Abuse Services

- Outdoor supplies

- Primary Health Care

- Case Management

- Basic Need Items

Outreach teams go out Mondays from 10am-12pm & Thursdays 2:30-5:30pm.

The Street Outreach team can meet you where you are at or you are welcome to visit us at our set sites listed below.

MONDAY

10 a.m. to 12 p.m.: Collaborative Mobile Outreach

10:30 a.m. to 12:30 p.m.: Mobile Food Truck and showers: Northwest Christian Church 714 Benson St.

TUESDAY 8 a.m. to 10 a.m.: Open Walk-in Case Management at New Visions

WEDNESDAY

1 p.m. to 3 p.m.: Open Walk-in Case Management at New Visions

10 a.m. to 12 p.m.: Housing & Employment Clinic Council Bluffs Public Library 400 Willow Ave.

4 p.m. to 6 p.m: Housing Clinic (assistance with housing applications, listings, etc.) Anawim 300 W. Broadway

THURSDAY

8 a.m. to 10 a.m.: Open Walk-in Case Management at New Visions

11 a.m. to 1 p.m.: Housing Clinic at HFS Peer Center 3415 W. Broadway

2 p.m. to 5 p.m.: Mobile Outreach

FRIDAY Open for follow-ups & referrals
