Take Healthy Action with Diabetes PATH Workshop: Presented by Tri-County Office on Aging [Reg. Req.]

What is it? Diabetes PATH: Personal Action Toward Health (Diabetes PATH) is a 6-week community diabetes education program to help individuals learn the tools and techniques needed for day-to-day management. The program was developed and rigorously tested by Stanford University.

Benefits: Research studies found that after the workshop participants had significant improvements in healthy eating, symptoms of hypoglycemia, communication with physicians, depression, and reading food labels. Participants also had improvements in patient activation and self-efficacy.

Who should participate: Older adults with diabetes, pre-diabetes, and/or caregivers of those with diabetes.

Workshop Features:

Led by trained leaders.

Meet for 2 ½ hours per week for 6 weeks.

Group size is approximately 8-12 participants.

Offered in convenient, easily accessible locations. An online option may be available.

Interactive style workshop where mutual support and success build the participant’s confidence in their ability to manage their condition.

Utilize the book “Living a Healthy Life with Chronic Conditions.”

Free to attend but donations are accepted.
